Eclipse Dance Academy – Model Consent & Release Form

About This Form

Before your child can take part in the upcoming photo shoot, we require a completed consent and model release form. This is a standard procedure for all our photography sessions and ensures we have your permission to photograph your child, to use selected images where appropriate, and to maintain a safe, professional environment.

The form contains three sections:

• Consent to Photograph

• Model Release

• Release of Liability

We kindly ask that you complete this form prior to the photo shoot so that everything is in place for your child to participate. Thank you for your cooperation.

Section 1: Consent to Photograph

I, the undersigned, am the parent or legal guardian of the child named below. I give permission for Jason Edwards at Elite Portrait Photography to photograph my child during the photography session listed above. I understand that the photographs will be used for professional purposes, and I confirm that I am legally authorised to grant this consent.

Section 2: Model Release (Eclipse Dance Academy)

I grant Lauren at Eclipse Dance Academy the irrevocable right to use, publish, and distribute the photographs taken of my child for any lawful purpose, including but not limited to:

• Display on Eclipse Dance Academy website

• Display on Eclipse Dance Academy social media

• Advertising

• Promotional materials

Section 3: Release of Liability

I understand that the photographer and the dance academy are not obligated to use any of the images.

I will not make any legal claims against the photographer for how the final images look, as long as they are used respectfully and according to this agreement.

I understand that the photographer and the dance academy will take all reasonable care throughout the photography session.
